A field that chooses a month and year from the twelve months of a year.

First version: choosing a month from a twelve-cell grid; min, max, isMonthUnavailable, placeholder, size, readOnly, name (YYYY-MM); the ui.month-picker.* strings.

Changed

size="sm" (32 px) is added; the trigger is 32, 36 and 40 px, as in Select.
Cells are 36 px tall with the option row corners (menu-item-radius); focus is a solid 2 px outline.
